Violation Description: Mrs X complained about the care package she received from the Care Provider. The Care Provider was not at fault in terms of consistency of the care package, or in how it investigated incidents when Mrs X raised them. The Care Provider did not keep proper records of some concerns and it was at fault in how it communicated the outcome of investigations. It did not respond properly to Mrs X's complaint. These faults caused uncertainty and distress for Mrs X. The Care Provider has agreed to apologise to Mrs X.
