Violation Description: On 3 August 2011, a deck hand working on the Woolwich Free Ferry sustained severe head injuries and drowned when he was dragged overboard by a mooring line while releasing lines that were securing the ship to a mooring buoy. An investigation opened by the Maritime and Coastguard Agency showed that the risk assessment and procedures in operation at the time had not recently been updated and did not cover the work being carried out. It was also found that the crew were not being monitored to ensure compliance with procedures.
