Violation Description: Mrs X complains there were failings in the care and support provided to her mother Mrs Y while at a care home run by the care provider causing pain and distress to Mrs Y and the family. We found fault as the care provider's documentation was not always accurate and complete. The care provider has accepted the errors in documentation and already apologised to Mrs X which is suitable action for it to take. So, we have completed our investigation.
