Violation Description: There was a failure to update Mr and Mrs B's care plans which is fault as it is not in line with relevant care regulations. There was also a failure to act in line with the Mental Capacity Act 2005. This caused Ms A avoidable distress for which the Care Provider will apologise. The Care Provider withdrew the service before the end of the period of notice set out in the contract. This is also fault, but caused no injustice because Ms A said she would have preferred to end the service immediately.
