Violation Description: Mrs X complains that the Care Provider failed to follow its policy and procedures when her late mother, Ms Y, fell. Five days later, Ms Y was in hospital with a fractured pelvis. The Ombudsman finds the Care Provider caused Ms Y an increased risk of harm, and distress to the family. The Care Provider will send the Ombudsman evidence of the action it took to avoid similar problems in future. It will also pay Mrs X £500.
